Permission to Pick-Up Scholar

I understand that my scholar may not leave campus during the school day without being signed out by one of the people listed on this form.
ANYONE who comes to sign out my scholar must show a photo ID and sign my scholar out in the main office.

If my scholar returns to school during the same school day, he/she must be signed back in in the main office by a person on this list.
